How to Refer a Patient to us:

1. Call us at 541-928-8341 to confirm availability of services. 
2. Inform your client of general ultrasound expectations, provide an estimate of cost and ensure transfer of patient arrangements have been made if necessary.
3. Fill out the Referral Form below with as much detail as possible. Include any relevant documents, including records, images, lab work, etc. if possible.
4. At intake the patient will be prepped and shaved for Ultrasound. Outpatient referrals do not require an exam by one of our DVM's unless sedation is required or significant medical event arises. If the stenographer feels there is anything significant found that needs immediate attention we will contact the referring hospital immediately at the number provided on the form.  
5. Once the Ultrasound has been completed your client will receive discharge instructions from one of our technicians. The report will be forwarded to the Hospital Email Provided once we have received it for you to review and follow up with your client.
